In HellThing the player controls Janus, a necromancer and sadistic psychopath whose life connects present-day 2025 with his gothic, emo-filled high school days in 2007. The narrative shifts between these periods as past events bleed into the present, forcing the uncovering of buried secrets while Janus gives in to his cruel impulses within a horror-tinged world.

Progression relies on player choices that create branching storylines. Each decision alters the direction of events and determines how relationships with a diverse cast of characters develop. The school setting of 2007 supplies the foundation for many situations, while 2025 brings those earlier threads to a head through gradual revelation and consequence.

Interactions incorporate corruption, mind control, BDSM, futanari, group sex, gender bender, and milf elements. These sequences appear through both 2dcg and 3dcg animations, allowing the player to experience the full range of graphic content tied to Janus’s desires.
